CONSULTATION FORM

Step One:

Please fill out this form if you are ready to make a free consultation appointment. We receive them via email and will respond to them in the order received in the most timely manner possible. Please do not fill out multiple forms for the same tattoo, it will delay the process. Complete all the areas with as many details as possible. This is the reference point we create your tattoo design from. You will be able to schedule a consultation time at the end of this form.

 

Step Two:

You will be contacted by studio staff or the artist listed via email or phone. During that correspondence a convenient free consultation appointment will be made. The personal consultation will be conducted either in person at the studio at a scheduled time or via phone/zoom. At the time of your visit the artist will go over all the information in the consultation form, answer any questions you have, collect your deposit, and schedule your appointment. All verification will be received via email.

Step Three-ish:

This is just deposit information. Please do not schedule a consultation if you are not ready to put down a deposit. No appointments will be scheduled with out an immediate deposit. Deposits vary from $100+ larger deposits for larger projects, half sleeve size or larger. They are non refundable but go towards the final cost of the tattoo.
